This form is to be completed if You have chosen your e-Gift Card and have downloaded it to your computer to insert it into this form. You wish to receive the e-Gift Card. It shows three prices: €49, €69, €99. Fill in the form that the Artist receives and sends back to you with all the necessary information in return. Indicate your name, first name and email address (buyer). Indicate if the e-Gift Card is in the Last Name or First Name of another person.

But yes, you can use all the advantages of this e-Gift Card for You, for a third party or for a personalized gift. It is the Name on the e-Gift Card that counts and will be used within six months and throughout the Art Gallery.

After having received Your e-Gift Card and Your payment, you will receive it in "return" with the necessary explanations and in the Name of the person you want It to be. Your e-Gift Card is valid for use within six months.
